This is for a normal adjustment visit for our regular patients.

Promotion Intake

For new patients only

Important:
1. For residents of the area only.
2. You will receive a confirmation email that contains a link to fill out your intake form. This must be filled out before you come to your appointment so we can get a good understanding of your health needs.
3. Our practice runs on time, so please arrive 10 minutes early to your first appointment so we can start on time.
4. If possible, consider wearing active wear that are relatively fitted, this is for this appointment only.

Child Intake

For new patients only

1. For the month of April, all Children Intakes is complimentary for our Kids Month!
2. You will receive a confirmation email with a link to an online form, please ensure that the form is filled out before your appointment so it can help us understand the health needs of your child.
3. Our practice runs on time, please ensure that you are 10 minutes early for your first appointment.

Tourist Adjustment

For new patients only

Important:
1. We love taking care of tourists traveling through our area who receive regular chiropractic care.
2. This means that it would be less than 6 months since you visited your regular Chiropractor and you have visit records and an X-ray report.
3. If this does not apply to you, please contact the practice so we can arrange the appropriate appointment.
4. You will receive a confirmation email that includes a link to a form, please ensure that you fill that out in advance so we have a good understanding of your health needs. If you have trouble filling out the form, please ensure that you arrive at the practice 10 minutes early for your appointment.
